Project Support Officer A life-changing opportunity Our client is the UK's leading dementia charity. They provide information and support, improve care, fund research, and create lasting change for people affected by dementia. Position: Project Support Officer Location: Lincoln (role covers south west Lincolnshire) Contract type: Fixed Term - 1 year contract Hours: 26 hours per week Salary: £18,145.03 actual per annum Closing date: 22 June 2017 Interview date: To be confirmed About the role: This is a twelve month project to support people with dementia to live well in their communities by enabling them to access appropriate services and support from their local General Practice and the wider community. The project will support the early diagnosis needs of people with dementia and enable practice teams to make changes within the practice to meet the needs of people with dementia and carers. This project will support existing activity towards the overall drive by South West Clinical Commissioning Group in Lincolnshire as part of their focus to increase diagnosis and post-diagnosis support. The main aims are to build on the successes of our client's evaluated models of support to Increase engagement and support for those worried about their memory and showing symptoms of having dementia, through piloted methods of community engagement with cohorts at risk of dementia. You will work with GPs and other health and social care professionals and community stakeholders to improve diagnosis and post-diagnosis support and ensure that people diagnosed have access to the Lincolnshire Dementia Family Support Service. About you: You will need excellent negotiating skills and experience of influencing others from across a section of the community. You will also need excellent communication skills and your willingness to travel independently to manage or promote your work.
